We dont check model releases, cool. It is the responsibility of the photographer to provide on for anyone wishing to use their image commercially. We ask you to check a box to declare you have one, and thats where our responsibility ends.
Im sure you could find a template online. I know I saw some years ago linked to one of the larger image sites. But there would be a generic one somewhere that you could edit and print off.
I just checked and there are lots. Ill have a look at your channel.
